Home Based Waiver Services (Traditional Home/Community & AWC/FMS Model)

The Human Resources Center, Inc. provides Home Based Waiver Services to eligible families. The intention of this program is to provide various supports to families with developmentally delayed children/adults living at home. The support services available through Home Based Waiver may include; temporary and overnight respite, transportation, adaptive equipment, physical adaptations to the individuals home or vehicle and companion and Home and Community Habilitation.

Eligable families in Carbon, Monroe, Pike and Wayne Counties.
