Home Based Services 

Human Resources Center, Inc. (HRC) is seeking self-motivated and compassionate Home-Based Direct Care Service Providers to provide Home and Community Habilitation, Temp Respite, and Transportation services.

The provider will assist the individual with developing healthy habits and social skills. Experience with intellectual disabilities and mental health diagnosis is strongly preferred.

There are several positions open with varying hours: part-time and full-time.

Minimum Requirements:

  • Must be at least 18 years of age

  • High School diploma or equivalent

  • Eligible to work in the United States

  • Consent to and satisfactorily pass pre-employment background checks, reference checks, and physical exams.

  • Possess a valid driver’s license.

  • Agree to carry out the responsibilities associated with individuals’ needs based on the Individual Support Plan (ISP), or other individual plans and company policies

Program Specialist – Home-Based Waiver ServicesPosition Summary

The Program Specialist for Home-Based Waiver (HBW) Services provides instructional, supportive, and administrative services to individuals and families enrolled in the program. This role is responsible for developing, coordinating, implementing, and monitoring Individual Support Plans (ISPs), while ensuring high-quality service delivery and compliance with regulatory requirements.

The Program Specialist works collaboratively with individuals, families, team members, and community partners to promote independence, enhance quality of life, and support individuals in achieving meaningful, self-directed outcomes within their home and community.

About Home-Based Waiver Services

HRC’s Home-Based Waiver Services provide personalized, in-home supports to children and adults with intellectual disabilities throughout Northeastern Pennsylvania. Services are designed to promote independence while maintaining the comfort and familiarity of home.

Through individualized planning and compassionate support, our team helps participants build daily living skills, engage in their communities, and achieve their personal goals.

Essential Duties and Responsibilities

  • Manage and maintain an assigned caseload of individuals enrolled in the HBW program

  • Develop, implement, and monitor Individual Support Plans (ISPs) in collaboration with individuals and their teams

  • Participate in ISP meetings and assist in developing annual goals and outcomes

  • Conduct home visits and complete timely, accurate case note documentation

  • Provide ongoing case management support to individuals and families

  • Coordinate services with internal departments and external providers

  • Review unit utilization and ensure services are delivered as authorized

  • Review and approve daily service documentation and case notes

  • Attend scheduled meetings and appointments as requested by individuals or families

  • Assist families in identifying and accessing resources (e.g., assistive technology, home modifications, camps, and other supports)

  • Support recruitment, interviewing, hiring, training, and supervision of Home-Based staff

  • Complete employee evaluations (90-day and annual)

  • Assist with disciplinary actions, including termination of staff when necessary

  • Ensure compliance with all agency policies, state regulations, and licensing requirements

  • Maintain accurate records and documentation in accordance with program standards

  • Participate in a minimum of 24 hours of annual training

Education and Experience

  • Bachelor’s degree from an accredited college or university (required)

Residential Services

Direct Support Professional 

We're currently seeking a dedicated Residential Direct Support Professional to join our team. In this role, you'll play a vital part in maintaining a safe and nurturing environment for our residents. Your primary responsibility will be to offer personalized support tailored to the unique needs and preferences of those under our care, with the aim of fostering independence, integration, and an enhanced quality of life. Please note that this position requires travel and/or transportation.

Position Requirements:

To excel in this role, you'll need to meet the following criteria:

  • An Associate Degree in a related field, or a High School Diploma/GED (experience preferred).

  • Possession of a valid PA Driver’s License with a clean driving record.

  • Preferred age of 21 years or older.

  • Clearances: PA Criminal Record Check, Child Abuse Clearance, and Federal Background Check with acceptable results.

  • Completion of a physical examination by an agency-approved physician with satisfactory results.

  • Successful completion of Medication Administration, First Aid, CPR, CPI, CPS, and any other assigned training within an acceptable time frame or as scheduled.

That's why we have been offering Lifesharing services for many years, pairing long-term living arrangements with host families who open their homes and hearts to these individuals. Our providers can be a single adult, couple, or family unit, and they may either rent or own their home. This is an incredibly rewarding opportunity to make a significant impact on someone's life, giving them a chance to thrive in a loving family environment.
