Home Based Services 

HBW Direct Care DSP

Human Resources Center, Inc. (HRC) is seeking self-motivated and compassionate Home-Based Direct Care Service Providers to provide Home and Community Habilitation, Temp Respite, and Transportation services to individuals in Wayne, Pike Carbon, Monroe, Lackawanna, and Susquehanna counties.

The Department covers Carbon County (Jim Thorpe, Albrightsville, etc.), Monroe County (Stroudsburg, East Stroudsburg, Tobyhanna, etc.), Pike County (Milford, Matamoras, Dingmans Ferry, etc.), Wayne County (Honesdale, Hawley, Waymart, etc.), Lackawanna County (Scranton, etc.), and Susquehanna County (Clifford Twp, etc.).

The provider will assist the individual with developing healthy habits and social skills. Experience with intellectual disabilities and mental health diagnosis is strongly preferred.

There are several positions open with varying hours: part-time and full-time.

Minimum Requirements:

  • Must be at least 18 years of age

  • High School diploma or equivalent

  • Eligible to work in the United States

  • Consent to and satisfactorily pass pre-employment background checks, reference checks, and physical exams.

  • Possess a valid driver’s license.

  • Agree to carry out the responsibilities associated with individuals’ needs based on the Individual Support Plan (ISP), or other individual plans and company policies

Assistant Director

Key Responsibilities:

Program Leadership and Development

  • Oversee daily operations with a focus on person-centered care, quality, safety, and programming.

  • Develop and grow services by building strong community networks and referral relationships.

  • Represent HRC through public speaking and community engagement to promote services and nurture partnerships.

  • Set and achieve program goals aligned with HRC’s strategic objectives and state regulatory guidelines (including Chapter 6100).

  • Facilitate innovative processes to enhance client care and satisfaction.

Team Management

  • Supervise and support Program Specialists, providing guidance, feedback, and problem resolution.

  • Conduct regular performance evaluations to identify areas for improvement or restructuring.

  • Ensure adherence to company policies, state regulations, and safety standards.

Financial Oversight

  • Monitor program performance from an operational and fiscal perspective.

  • Collaborate with administration to ensure long-term financial stability.

Compliance and Reporting

  • Review internal audits conducted by Program Specialists and address any areas of concern.

  • Keep senior management updated on program developments and challenges.

General Requirements

  • Maintain a consistent work schedule with frequent travel to service locations.

  • Adhere to HRC policies, procedures, and all applicable laws and guidelines.

  • Perform other duties as assigned.

Qualifications:

  • Bachelor’s degree in a related field from an accredited institution.

  • At least 3 years of experience working directly with individuals with intellectual disabilities.

  • Supervisory experience is required.

Community Participation Day Services

CPS Direct Support Professional

Direct Support Professional – Community Participation Supports (CPS)

At The Human Resources Center, Inc., Direct Support Professionals (DSPs) provide personalized support to individuals with disabilities, fostering independence, community engagement, and improved quality of life. This full-time role involves assisting with daily care, facilitating activities, and promoting inclusion through meaningful participation in both facility and community settings.

Location: Church Street
Schedule: Monday-Friday, full-time
Pay: Starting at $15/hour based on experience

Position Highlights

  • Support individuals with personal care, meal prep, and medication administration.

  • Foster job skills through volunteer opportunities.

  • Facilitate recreational and social activities.

  • Maintain safety in facility and community settings.

  • Transportation and travel required.

Qualifications:
High School diploma or GED, valid PA driver’s license, required clearances, and completion of training certifications.

Lifesharing Coordinator

Join Our Management Team!

Make a meaningful impact as a Lifesharing Coordinator in our Intellectual and Developmental Disabilities (IDD) Residential Services Division. This role focuses on fostering family-like environments for individuals with intellectual disabilities and/or Autism through our Lifesharing program.

Key Responsibilities:

  • Recruit, train, and evaluate Lifesharing providers.

  • Ensure compliance with state and agency regulations.

  • Support individuals in achieving personal goals and community integration.

  • Collaborate with families, providers, and agency staff to ensure program success.

Qualifications:

  • Bachelor’s degree in human services or related field (Master’s preferred).

  • 2+ years in the IDD or human services field.

  • Strong organizational, communication, and interpersonal skills.

  • Valid driver’s license and regional travel required.
