Position Summary
The Human Resources Center (HRC) is seeking a compassionate and organized Program Specialist to support individuals, older adults, and families receiving Home-Based Services & Eldercare services. This position is responsible for coordinating services, supporting consumers and caregivers, maintaining regulatory compliance, and ensuring high-quality, person-centered care that promotes independence and community living.
Key Responsibilities

Coordinate and monitor home-based services for consumers, including older adults participating in Eldercare programs.

Conduct assessments, maintain documentation, and support individualized service planning.

Collaborate with consumers, families, caregivers, referral sources, and community partners to ensure effective service delivery.

Assist with staff recruitment, scheduling, training, supervision, and ongoing support.

Monitor program compliance with agency policies, licensing standards, and funding requirements.

Review service documentation, billing information, and program records for accuracy and completeness.

Participate in audits, meetings, trainings, and quality improvement activities.
Qualifications

Bachelor's degree in Human Services or a related field preferred; equivalent education and experience considered.

Experience working with older adults, individuals with disabilities, human services programs, case management, healthcare, or community-based services preferred.

Strong organizational, communication, and problem-solving skills.

Valid driver's license and ability to travel throughout the service area.

Ability to obtain and maintain all required clearances and certifications.
Home Based Services
Human Resources Center, Inc. (HRC) is seeking self-motivated and compassionate Home-Based Direct Care Service Providers to provide Home and Community Habilitation, Temp Respite, and Transportation services.
The provider will assist the individual with developing healthy habits and social skills. Experience with intellectual disabilities and mental health diagnosis is strongly preferred.
There are several positions open with varying hours: part-time and full-time.
Minimum Requirements:

Must be at least 18 years of age

Consent to and satisfactorily pass pre-employment background checks, reference checks, and physical exams.

Possess a valid driver’s license.

Residential
As a Direct Support Professional (DSP) at the Human Resources Center, Inc. (HRC), you will be a cornerstone of support for individuals with disabilities, fostering their independence, community participation, and overall quality of life. This role includes personalized, one-on-one assistance in both residential and community settings, emphasizing inclusion and skill-building.
Position Requirements

Education: High School diploma or GED (1-2 years of experience preferred but not required).

Age: Recommended 21 years or older.

Driver’s License: Valid PA license with an acceptable driving record.
-
Clearances: PA Criminal Record Check, Child Abuse Clearance, and Federal Background Check with acceptable results.

Physical Requirements: Must pass a physical examination upon hire and every two years thereafter.

Training: Complete Medication Administration, First Aid, CPR, CPI, and CPS courses within required timeframes.
Key Responsibilities
Daily Support:

Assist individuals with personal hygiene, meal preparation, medication administration, and ambulation.

Community Engagement:

Support participation in recreational and volunteer activities based on individual preferences.

Safety and Supervision:

Ensure a safe environment at all times, including emergency evacuations if needed.

Monitor meals and breaks to mitigate safety risks.
Medication Administration:

Administer prescribed medications following training and protocols.
Transportation:

Serve as a driver or aide on agency vehicles, completing all required documentation.
Physical Demands

Ability to lift or transfer up to 50 lbs. using proper techniques.

Frequent physical activity, including standing, walking, and assisting with exercises.
