Home & Community Based Services
The Human Resources Center, Inc. presently provides licensed "Group Home" services to adults within Lackawanna, Susquehanna, Wayne, Carbon, Monroe and Pike Counties in Pennsylvania. Each home provides: 24 hour supervision with trained agency staff, individualized service planning, assistance in all aspects of daily living, coordination of medical services, transportation, involvement in community integrated activities as well as a multitude of other services.
Functioning like any other home, residents live, work, attend day program, go on outings to shows, shopping etc., while at the same time receiving assistance and support .
Home Based Waiver Services are provided to eligible families. The intention of this program is to provide various supports to families with intellectually disabled children/adults living at home. The services available through Home Based Waiver may include: respite, transportation, adaptive equipment, physical adaptations to the individuals home or vehicle, companion, in-home and community supports.
Life Sharing is a unique residential option for individuals with intellectual disabilities. The service is similar to foster care where placement with a family occurs, however there are some differences. Life Sharing is viewed as a permanent life long placement that can occur at any age. Families that become involved in Life Sharing are closely screened, trained and supervised. Individuals referred to this service are matched with families whose life style and interests are similar to their own. Through Life Sharing we help find permanent homes for individuals where they can be integrated into the family unit, share their hopes, dreams and desires and live a happy and meaningful life..
A companion program for intellectually disabled individuals who reside in nursing homes, to compliment the services they offer. Individuals are often taken shopping, to movies, attend community events, visit relatives and /or are afforded time in a private "one to one" setting. This service enhances the quality of life for individuals who otherwise may lead lonely existences.
